Please call 802-859-1577 ext 400 to schedule your initial intake appointment with our Clinical Director, Suzanne Adams, to discuss treatment options with you.

After this assessment and the evaluation is completed, Suzanne will refer you to one or more of our providers at The Adams Center. Together, you, Suzanne, and your providers will formalize a more complex treatment plan, based on your needs.

INTAKE EVALUATION

During your intake appointment with our Clinical Director, Suzanne Adams, you will co-create a plan utilizing traditional, evidenced based therapies, as well as appropriate complementary therapies, such as energy work and body work.

Suzanne works with your primary care physician, as well as the providers at The Adams Center, to coordinate services.  Every treatment plan is uniquely customized based on background and experience, as well as your individual needs.  All of the professionals at our Center are dedicated to supporting you in your recovery.

We will check your benefits with your insurance company for the services offered here and have that information prepared at the time of your assessment appointment with Suzanne Adams.

GOOD FAITH ESTIMATE Notice

If you are not using insurance, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including psychotherapy services.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
