NEW ADMISSIONS
Please call 802-859-1577 ext 400 to schedule your initial intake appointment with our Clinical Director, Suzanne Adams, to discuss treatment options with you.
After this assessment and the evaluation is completed, Suzanne will refer you to one or more of our providers at The Adams Center. Together, you, Suzanne, and your providers will formalize a more complex treatment plan, based on your needs.
INTAKE EVALUATION
During your intake appointment with our Clinical Director, Suzanne Adams, you will co-create a plan utilizing traditional, evidenced based therapies, as well as appropriate complementary therapies, such as energy work and body work.
Suzanne works with your primary care physician, as well as the providers at The Adams Center, to coordinate services. Every treatment plan is uniquely customized based on background and experience, as well as your individual needs. All of the professionals at our Center are dedicated to supporting you in your recovery.
Admissions – Intake Evaluations with our Clinical Director
There is a one time $45.00 registration fee that is payable by cash, check, or credit card, due at the time of your Intake Evaluation with Clinical Director, Suzanne Adams, M.A., Licensed Psychologist-Master. (This fee is in addition to the telehealth charge for your intake evaluation, which will be billed to your insurance company.)

We will check your benefits with your insurance company for the services offered here and have that information prepared at the time of your assessment appointment with Suzanne Adams.
